MN Dept. of Education Supplementary OLL Form - Required
It may take 7 to 10 days to complete an enrollment after ALL paperwork and payment (if required) has been received.
MS Word Version (192 KB, requires MS Word - can be downloaded and completed electronically before printing)
Students: Please click on one of the links above to download this form. Then print the form and complete the first page of the form and return it to MSC Online.
School Districts: When you receive this form from either MSC Online or your student, please complete and sign the school district sections on page two of the form, including indicating if the course(s) requested will meet the student's graduation requirements or academic plan (check and initial). Then return the form to MSC Online.
PLEASE NOTE: Students taking fully online courses through MSC Online are required to have an email address upon registration. This can be a school account or a free web-based account like Gmail.
